Sacramental / RCIA Online Registration
Welcome to the Sacramental / RCIA Program at MSLRP.
 Parents, do you have your parish registration number handy? If you don't have one,
 please hit the back button on your browser and go fill out our parish registration form.
 Then call the front office at 760-757-3250 for your number. You'll need that number in
 order to fill out your online Faith Formation registration form.
 By filling out this form and clicking SUBMIT, you authorize your child to participate in
 the Faith Formation program of this parish. You also give your consent for your child
 to receive first aid and / or 9-1-1 medical treatment. Please be sure to indicate in the
 remarks if your child has any Allergies or medical condition (Diabetes etc.), takes any
 medications, or has any Special Needs (Autism, ADD, ADHD, Downs, a Learning
 disability).

 Please be sure to include any sacraments your students have received along with the
 dates. Thank you for providing complete information.
